Video/DVD Reservation Form

Class Reserve Form
The Video/DVD Reservation Form is provided for faculty/staff to request materials for classroom use. Items will be picked up at the Learning Resource Center on the sixth floor of Milner Library and returned to the Learning Resource Center. The loan period for materials is a maximum of one week. The Class Reserve Form is provided for faculty/staff to place videos, DVDs, CDs, recordings, scores and other non-print items on reserve at the Learning Resource Center for a particular class. Items placed on reserve are for student use in Milner Library.
